Assumption of Risk:

I, the undersigned, acknowledge that participation in activities at FUN HOUSE (FUN HOUSE MANAGEMENT COMPANY LLC) at 212 Greece Ridge Center Dr, Rochester NY 14626, involves inherent risks, including but not limited to physical injury, property damage, or other harm. Those risks could lead to injuries including but not limited to bruises, sprains, broken bones, serious injury, paralysis, or death, as well as property damage to me or others. I voluntarily assume all risks associated with participation in these activities.

Release and Waiver of Liability:

In consideration of being allowed to participate in activities at FUN HOUSE (FUN HOUSE MANAGEMENT COMPANY LLC), I hereby release, waive, discharge, and covenant not to sue FUN HOUSE (FUN HOUSE MANAGEMENT COMPANY LLC), its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me or my property while participating in activities at the facility.

Medical Treatment:

I permit FUN HOUSE (FUN HOUSE MANAGEMENT COMPANY LLC) to seek medical treatment for me in case of injury, accident, or illness during my participation in activities. I agree to be responsible for any and all costs associated with such treatment.

Indemnification:

I agree to indemnify and hold harmless FUN HOUSE (FUN HOUSE MANAGEMENT COMPANY LLC), its owners, employees, agents, and representatives from any claims, actions, damages, costs, and expenses, including attorney's fees, arising from or related to my participation in activities at the facility.

Photography and Videography:

I understand that FUN HOUSE (FUN HOUSE MANAGEMENT COMPANY LLC) may take photographs or videos of participants during activities for promotional or marketing purposes. I grant permission for my likeness to be used in such materials without compensation.

I acknowledge and understand that Fun House is not a Day Care, and that it is my full responsibility to supervise all children named above at all times.

I, as well as my spouse, my parents, any other accompanying adult, and all children named below, will comply with all rules of Fun House, including posted signs and verbal instructions from Fun House Staff.

Parent/Guardian Consent:

I, the undersigned, on behalf of myself, my spouse, my parents, any other accompanying adult, and all children named have read and understand this waiver and agree to all terms on behalf of the participant valid through  the year 2024.
